RI Dept. of Health Claims That Two Health Professionals “Committed Fraud”
Nursing assistants Annmarie Hernandez and Guadalupe Da Silva were reprimanded by RIDOH this week for lying about showing up to work, when they did not do so.

Reprimands for False Documentation
According to RIDOH, Hernandez was employed as a nursing assistant “by a staffing agency and was assigned to work at the home of a patient...on August 21, 2024.”
“[On August 21, 2024] she failed to appear at the home of a patient at 4:30 PM as scheduled, yet clocked in at the patient’s home at 4:27 PM, when in fact she did not arrive at the patient’s home until approximately 5:30 PM, thus committing fraud," wrote RIDOH.
In the matter of Da Silva, RIDOH similarly found “unprofessional conduct.”
“On August 14, 2024, Da Silva was employed as a nursing assistant by a staffing agency and assigned to work at the home of a patient,” wrote RIDOH.
In that instance, RIDOH said, “Da Silva notified the staffing agency that she had arrived at the home of the patient when in fact she had not arrive…thus committing fraud.”
