GoLocal has reviewed federal data and identified the 15 nursing homes that have been fined more than $100,000 in the most recent data released by the Centers for Medicare and Medicaid Services (CMS).

The national Centers for Medicare & Medicaid Services' (CMS) data was downloaded by ProPublica, and GoLocal evaluated and ranked the state’s nursing homes by the number of fines. The data reported covers the last four years and was most recently updated in February of 2026.

Of the 15 nursing homes fined $100,000, three of those homes were issued RED FLAGS 🚩 by the federal government.

Red Flags identify a Special Focus Facility: Flagged by the government for having a history of serious quality issues.

The only good news is that since GoLocal did this review in 2024, the number of homes fined $100,000 or more decreased from 18 to 15.
