GoLocalProv News Team
GoLocal reviewed and scored the data collected by the RI Department of Health for health inspections of school cafeterias across Rhode Island for the past two years. This marks the second time GoLocal has analyzed the data.
In February 2015, GoLocal found that the schools with the highest number of violations over a multi-year span were Westerly High School with 52, South Kingstown with 43, and Providence's Mount Pleasant High School with 42.
Now, see which schools showed improvement and the ones the racked up significant numbers of violations.

Inspector Comments
Thermometers are lacking in the refrigeration units. Cold holding equipment used for potentially hazardous food must have at least one thermometer that is located to allow easy viewing of the temperature display.

Inspector Comments
Prepackaged meats were observed thawing on a rack in the hallway. Potentially hazardous foods must be thawed in one of the following ways: under refrigeration (41 degrees F or less), completely submerged under running water, as part of the cooking process, or in the microwave if cooked immediately after thawing. Rack was relocated to the walk-in cooler during inspection.

Raw shelled eggs located in the refrigerator had expired per the expiration date (August 21). Eggs were voluntarily disposed during inspection.

The sneeze guard on the salad bar is not an appropriate height for the children being served, it needs to be lowered. All fruits and vegetables on the salad bar must be wrapped.

Inspector Comments
Old mouse droppings were observed in the dry storage area, located near a baiting station by the boiler. Pictures obtained. Per PIC, facility has monthly PC service, and a technician is scheduled to come today. Person in charge must follow up with inspector.

Inspector Comments
Cookie dough and pretzels were observed thawing in cooling racks at room temperature. Potentially hazardous foods must be thawed in one of the following ways: under refrigeration (41 degrees F or less), completely submerged under running water, as part of the cooking process, or in the microwave if cooked immediately after thawing. Cookie dough and pretzels were relocated to reach-in refrigerator for proper thawing.

The presence of rodents is not controlled as evidenced by observed mouse droppings & urine seen in various areas throughout the establishment. The presence of insects, rodents, and other pests must be controlled to minimize their presence on the premises by routinely inspecting the premises for evidence of pests, using acceptable methods to control pests, eliminating harborage conditions and routinely inspecting incoming shipments of food and supplies. School Administrators, Aramark on site during inspection to collectively coordinate resolution to resolve problems expeditiously.

The previously cleaned deli slicer had an accumulation of food and debris along the back of the blade. The food contact surfaces of equipment must be clean to sight and touch. Inspector reviewed proper cleaning methods and associated risks of deli slicers. Deli slicer cleaning document was provided to PIC. Deli slicer was re-cleaned during time of inspection.

Droppings observed in areas throughout the kitchen area (shelves, floor, sill, etc.) and the vicinity of the area (underneath stage and in dining area). Indicators of significant rodent issue observed. Location to address issue immediately. Pictures obtained.

Inspector Comments
Chili and cooked ground beef which were prepared in advance [yesterday], were not cooled to 70 degrees in 2 hours and then to 41 degrees F within 4 hours. Chili and ground beef were observed in covered metal pans, stacked on top of one another in the walk-in cooler between 45 and 47 degrees F. Chili and ground beef observed warmer in the center [45-47 degrees F], and cooler around the edges of the pan. Employee stated chili had reached 70 degrees F within two hours of cooling; chili was then covered and stored in the walk-in cooler to complete the cooling process. Voluntary disposal completed. Proper cooling methods were reviewed. Proper cooling log documentation was reviewed.

The floor and wall around the dish washing machine is not clean to sight. The floor in both walk ins are in poor repair. The wall behind the mixer is not clean to sight. There are wooden ramps going into the walk ins that are not clean to sight and are not an easily cleanable material.

(cheese bacon quesadilla) was held at ( 110 ) degrees. Potentially hazardous food must be maintained at 135 degrees F or above except during preparation, cooking or cooling. Food was delivered hot from the WMS. Temperature upon arrival was 110 degrees. Corrective action was to reheat product to 165 degrees. Verified by inspector.

Buffalo chicken and sliced tomatoes were held between 46-50 degrees F on the salad bar. Food was out for one hour and temperature logs indicate that food was 40 degrees F prior to putting it out for service. PIC stated that all meats are disposed of after lunch and the vegetables are cooled down to 41 degrees F. Salad bar is not designed to maintain food temperatures. Ice packs are placed underneath pans but food products are not maintaining 41 degrees F or below. Potentially hazardous food must be held at 41 degrees F. or below except during preparation, cooking or cooling.

Cole slaw (51 degrees) and string cheese (55.6 degrees F) was received from Woonsocket High School out of temperature during inspection. Food was voluntarily disposed. Person in charge to monitor receiving temperatures.
